Seuls les services psychiatriques (médecins) sont remboursés par l'assurance publique, ainsi que les consultations psychologiques dans les établissements publics (CLSC, hôpitaux, cliniques communautaires), où les délais dépassent régulièrement 6 à 18 mois.
En pratique, 80 pour cent des Canadiens accèdent aux soins psychologiques via leur assurance collective d'employeur ou une assurance privée individuelle. Les nouveaux arrivants et expatriés sous permis de travail bénéficient généralement d'une couverture santé collective incluant entre 500 et 5000 dollars annuels pour les services psychologiques, selon l'employeur. Si vous êtes travailleur autonome ou en attente de résidence permanente, souscrivez une assurance privée auprès d'assureurs comme Manulife, Sun Life ou Canada Vie qui proposent des forfaits psychothérapie. Les étudiants internationaux disposent souvent d'un régime universitaire couvrant 500 à 1500 dollars par année académique. Enfin, certains organismes d'aide aux immigrants comme le COSTI à Toronto ou le PROMIS au Québec offrent quelques séances gratuites ou à tarif réduit en français, mais avec disponibilité très limitée.
Privilégiez d'abord un psychothérapeute inscrit auprès de l'ordre professionnel de votre province : Ordre des psychologues du Québec (OPQ), College of Psychologists of Ontario (CPO), College of Alberta Psychologists (CAP) ou équivalent. Au Québec, le titre de psychothérapeute est également protégé depuis 2012 et requiert une inscription à l'Ordre des psychologues ou à un ordre professionnel habilité (travailleurs sociaux, conseillers d'orientation, psychoéducateurs). Vérifiez le numéro de permis sur le site de l'ordre concerné pour confirmer que le praticien est en règle et sans sanction disciplinaire.
Au-delà des qualifications réglementaires, évaluez l'adéquation culturelle et thérapeutique : le thérapeute a-t-il une expérience avec les enjeux d'expatriation, d'adaptation culturelle, de deuil migratoire ? Pratique-t-il l'approche thérapeutique qui vous convient (cognitivo-comportementale pour l'anxiété et la dépression, psychodynamique pour un travail en profondeur, EMDR pour les traumas) ? La qualité de la langue française compte aussi : certains praticiens bilingues parlent français couramment mais ont été formés en anglais, ce qui peut influencer le vocabulaire clinique. Consultez les avis, demandez une première rencontre exploratoire (offerte par 70 pour cent des thérapeutes au Canada), et n'hésitez pas à changer si l'alliance thérapeutique ne s'établit pas après 2 à 3 séances. Enfin, clarifiez dès le départ les modalités pratiques : tarif exact, politique d'annulation (généralement 24 à 48 heures de préavis), fréquence recommandée et durée prévisible du suivi.
